Getting started

We accept a referral or your direct interest. We schedule a welcome call to learn goals, routines, support needs, and preferences. We confirm eligibility and service hours with your case manager.

Assessment and match

A bttr. assessor meets with you to understand strengths and support needs. We match you with a coach who fits your schedule and priorities. We set a start date and communication plan.

Plan your path

We build your Individual Service Plan with clear goals, activities, and target outcomes. You choose focus areas from our six bttr. skill sets. You approve the plan with your family and case manager.

Start coaching

Your coach meets you at home and in the community. Sessions include real tasks, real practice, and consistent feedback. We log notes and track progress after every visit.

Review and grow

We review progress monthly and quarterly. We update goals when skills improve or needs change. Families and case managers receive concise summaries and next steps.

Long term support

We celebrate milestones and plan what comes next. You may continue ILS, adjust hours, or transition support based on your goals. Our team stays available for check ins.

First 30 days

Week 1

Welcome call, intake, consent forms, and schedule setup. Safety review and communication preferences. Household and community orientation.

Week 2

Assessment visits and baseline skills check across LIFE, MONEY, TECH, HEALTH, EXPRESS, and CONNECT. Draft goals and activities.

Week 3

Plan review with you and your circle. Confirm schedule, locations, and priority tasks. Coach match confirmed.

Week 4

Coaching starts with measurable actions and simple trackers. First progress summary sent to family and case manager.

Scope and safeguards

What ILS includes

Coaching for daily living skills, community access, health routines, scheduling, organization, and self advocacy. Support during tasks and appointments as part of skill practice.

What ILS does not include

Emergency medical transportation or emergency on demand response. Coaches contact emergency services when needed and follow safety plans. We help plan non urgent transportation and backup options.

Communication standard

We confirm schedules in advance. We send monthly summaries to families and case managers. We reply to messages within one business day.

Where do sessions happen

At home and in the community based on your plan. Parks, libraries, stores, and other places you choose.

Who can join meetings

You decide. Family and supporters are welcome. Your case manager can join reviews when needed.

Can I start through Self Determination

Yes. We support participants funded through the Self Determination Program and through Regional Center vendor services.
